Diploma Replacement and Certified Copy Instructions

GENERAL INFORMATION

Replacement or additional diplomas will be issued to Ohio State graduates upon written request to our office. To obtain a duplicate/replacement diploma, the graduate must submit a notarized Duplicate/Replacement Diploma Request Form and either email it to board@osu.edu or mail it to:

Certified copies of an Ohio State diploma can also be requested through this office. Please send a clear, legible photocopy of your diploma to board@osu.edu, and we will certify and forward it per your instructions. If you would like to obtain a certified copy in person, you may bring a photocopy of your diploma to our office at the address above, Monday-Friday, between 9:00 a.m. - 5:00 p.m. If you plan to stop by our office, it would be advised to call ahead and make sure a notary is going to be available.

Fees are as follows: $15.00 per duplicate/replacement diploma and $9.00 total for domestic shipping; $3.00 per red diploma cover, upon request; and $2.00 per certified copy, upon request. For international shipping, please email board@osu.edu with the recipient's mailing address to receive a quote.

We accept several forms of payment. If you are mailing your request form to us, please send a check* or money order (made payable to The Ohio State University). Cash will be accepted from walk-ins only. We also accept most major credit cards (i.e., Visa, Mastercard, American Express, Discover Card, Diners Club and JCB). If you note that you want to pay via credit card, we will accept your payment details in person or over the phone.

STANDARD DIPLOMA REPRINTS

Q: I recently graduated and never received my diploma. How do I obtain it?
A: If you graduated within one year of the university’s most recent commencement ceremony and you never received your diploma, please contact the Office of Commencement and Special Events at commencement@osu.edu or (614) 292-9051. If you graduated more than a year before the university’s most recent commencement ceremony, you will need to submit a Diploma Request Form to board@osu.edu and we will assist you.

Q: The diploma I received at commencement was incorrect. How do I get a replacement?
A: Diplomas will be corrected for one semester following commencement. After that, diplomas must be corrected at the graduate’s expense. Q: Can I request a duplicate/replacement diploma for a family member, friend or potential employee?
A: Due to privacy laws, only the degree recipient can request a new diploma. Exceptions are dealt with on a case-by-case basis. Please contact us at board@osu.edu with questions.

Q: Does my Diploma Request Form have to be notarized?
A: If you are currently living in the United States, you must have your form notarized. If you are currently living overseas and do not have access to a notary, we will accept a copy of your government-issued photo ID (e.g., passport, driver’s license) instead.

Q: How do I submit my Diploma Request Form?
A: The quickest way to submit your form is by sending a scanned copy via email to board@osu.edu. (Make sure to include a scanned copy of your ID if you are overseas and unable to access a notary.) You can also send your request form in the mail to our office at:

Q: What happens after I submit my request?
A: Once we have confirmed that your request form is filled out correctly and have received payment, we will order your new diploma. Our diploma vendor will print it at an off-site facility and then deliver it to our office. We will contact you to let you know it is ready for pick up or that we have placed it in the mail. If you paid for the diploma to be shipped, we will email you a tracking number as well.

Q: How long does it take?
A: On average, it takes about two weeks from the time we place your order until you receive your diploma in the mail. If your request is urgent, we can send you a PDF copy of your diploma before we place it in the mail. However, you will need to request this as it is not standard procedure.

Q: Can a friend or family member pick up my diploma for me?
A: Yes, you can designate someone to pick up your diploma on your behalf. However, you must notify our office ahead of time and tell us the name of the person you have authorized to pick up your diploma. That individual will be required to show a photo ID upon pickup.

Q: How do I obtain a red cover for my diploma?
A: We sell the same red diploma covers that you would have received at commencement. They cost $3 each. If you recently graduated and did not receive a red diploma cover when your diploma was mailed to you, please contact the Office of Commencement and Special Events at commencement@osu.edu or (614) 292-9051 to request a padded cover at no charge.

Q: What size is my diploma? Can you frame it?
A: The Board of Trustees office does not offer framing services. However, diploma frames can be purchased at the official university bookstore, Barnes and Noble at The Ohio State University, or at other bookstores in the campus area. Diplomas are 8.5” by 11” and are oriented in a landscape printing position.


CERTIFIED DIPLOMA COPIES

Q: What is a certified copy of my diploma?
A: Some employers and higher education institutions may request a certified copy of your diploma for validation. On the back of a photocopy of your original diploma, we print a statement that acknowledges it to be an authentic copy of a degree from The Ohio State University. The statement is signed by the secretary of the Board of Trustees and notarized for authenticity.

Q: How do I get a certified copy of my diploma?
A: To request a certified copy of your diploma, please email us or bring to our office a photocopy of your diploma. Please ensure that your photocopy is clear and legible. If it doesn’t meet our standards, we will ask you to rescan the diploma and send it again. We will print the certification statement on the back of your photocopy and get it signed/notarized.

Q: How long does the certification process take?
A: So long as a notary is available in our office, the certification process is very quick. However, if a notary is not available, we will have to wait until they return to complete your certification. If you plan to come into our office to request a certified copy, it would be advised to call ahead and make sure a notary is going to be available.

Q: Can you certify my original diploma rather than a copy?
A: Yes, we can certify your original diploma. However, please double check that you need a certified original and not a certified copy. We most frequently see requests for copies, not originals, to be certified.   

Q: What if I need a certified copy, but I no longer have my diploma?
A: If you do not have your original diploma, you will have to submit a Diploma Request Form and order a new one through our office. We will make a copy of that diploma and certify it. Please note, you will be charged for both the replacement diploma and the certification.


APOSTILLES & AUTHENTICATIONS

Q: What is an apostille?
A: When certain public documents are to be used outside of the United States, they may require an apostille or other authentications, which validate official signatures. An apostille is an additional certification for documents that are going to be used in countries that are members of the 1961 Hague Convention. If the country is not a part of the Hague Convention, your document will need a gold seal authentication instead.

Q: How do I obtain an apostille or authentication?
A: Our office does not provide these additional authentications. After we certify a copy of your diploma, you will need to submit it to the Ohio Secretary of State. For more details, please read our quick overview on the Authentication of Academic Credentials.


NAME CHANGES

Q: I recently graduated from Ohio State and my name is incorrect on my diploma. How do I get a replacement?
A: Diplomas will be corrected for one semester following commencement. After that, diplomas must be corrected at the graduate’s expense. Q: I graduated from Ohio State more than a year ago and my name has since changed. How do I get a new diploma reflecting this change?
A: Please fill out a Former Student Request for Change of Record and a Diploma Request Form (which will need to be notarized). The Change of Record form lists the documentation you will need to provide, depending on why your name was changed. After filling out these forms and collecting the required documentation, please send all of your materials to us via mail, email or by stopping into our office. Once we receive your information, we will share it with the University Registrar’s office and process your diploma request.

Q: How do I know if my name change has been processed with the university?
