Diploma Replacement and Certified Copy Instructions

Replacement or additional diplomas will be issued to graduates upon written request to the Office of the Board of Trustees at The Ohio State University. To request a diploma, the graduate must submit a notarized duplicate/replacement diploma request form.
Name changes can be made by returning the original diploma and the request for change of records form to the Office of the Board of Trustees at the address listed above. Legal documentation of your name change is required. As a courtesy, our office will submit the completed change form to the university registrar's office.
The new diploma will be mailed to you via UPS in which a signature is required upon receipt. Please allow two weeks for processing and shipping of your request.
Note: Domestic shipments will require a signature upon delivery. International shipments will not require a signature upon delivery.
Certified copies of your OSU diploma can be requested through this office. Please forward the photocopy of your diploma to our office at the address above and it will be certified and forwarded per your instructions. If you would like to obtain a certified copy in person, you may bring a photocopy of your diploma to our office at the address above, Monday-Friday, 9:00 a.m. - 5:00 p.m.
Fees: $15.00 for the diploma and $9.00 for domestic shipping; $3.00 for a red diploma cover, if requested; $2.00 per certified copy (graduates must supply the photocopy). For international shipping, please email firstname.lastname@example.org with the mailing address to receive a quote.
Forms of Payment: We accept checks* and money orders (made payable to The Ohio State University) and most major credit cards (Visa, Master Card, American Express, Discover Card, Diner's Club, and JCB). Cash will be accepted from walk-ins.
*Please note that we do not accept counter checks, starter checks, third party checks, and foreign checks as sources of payments in this office.
