Replacement or additional diplomas will be issued to graduates upon written request to the Office of the Board of Trustees at The Ohio State University. The request for this diploma must be in the form of a notarized letter written by the graduate and should include the year (and term, if possible) of graduation, as well as the type of degree earned, and contact information including your return address, phone number, and email address.

If the reason for the replacement is a name change, please return the original diploma and a copy of the legal document showing the name change. Your official university records will be changed to reflect the new name.

The new diploma will be mailed to you via certified mail, usually within two weeks of the receipt of the request.

You should always maintain your original diploma. If you need a certified copy of your OSU diploma to provide to government agencies, employers, etc., please forward the photocopy of your diploma to our office at the address above and it will be certified and forwarded per your instructions. If you would like to obtain a certified copy in person, you may bring a photocopy of your diploma to our office at the address above, Monday-Friday, 8:00 a.m. - 5:00 p.m. (7:30 a.m. - 4:30 p.m. during Summer Semester).

Fees: $15.00 for the diploma and $9.00 for domestic shipping; $3.00 for a red diploma cover, if requested; $2.00 per certified copy (graduates must supply the photocopy). For international shipping, please email brady.114@osu.edu with mailing address to receive a quote. Checks/money orders should be made payable to The Ohio State University. Cash will be accepted from walk-ins. We do not accept credit card payments in this office.
